Leveraging PGHD to Improve Patient Care

Patients meet infrequently with their physician which means the information the care team is using typically is based on a small snapshot of the patient’s life.  Patient Generated Health Data (PGHD) can help address this issue. This emerging area is undergoing tremendous flux and there is little or no standardization related to how the data is collected and how it can be used to improve care. This is the first of several articles that will help readers develop a clear picture of what is required for PGHD to efficiently improve patient outcomes.

This is the second article in the Patient Generated Health Care (PGHD) series that defines the principles needed to help a diverse set of patients realize significant improvements in their care on a daily basis. The principles discussed the next two articles in this series are divided into three general categories related to 1) Clinical Usefulness: making it easier for clinicians to manage patient care, 2) Patient Experience: improving a patient’s experience related to the care they receive and 3) Outcomes and Cost: reducing the overall cost of care through improved outcomes.

Attached is a link to a whitepaper published on the HealthIt.gov website which is the  official website of The Office of the National Coordinator for Health Information TechnologyThis paper is titled “Conceptualizing a Data Infrastructure for the Capture, Use, and Sharing of Patient-Generated Health Data in Care Delivery and Research through 2024”.  The ONC went through a selection process and picked two companies to pilot the latest PGHD technology.  Each organization was paired with a health delivery organization.  See the section beginning on page 36 of this article titled “TapCloud/AMITA health”.  TapCloud is the tool that is currently the top choice of the VoiceOfThePatient members for working with their care teams to manage their medical condition.
